Consultation Terms & Conditions.

The Owner wishes to engage Behavet Ltd, and Behavet Ltd agrees to undertake the services under the terms in this Services Contract, as well as the Primary Consultation Form Above which will all become part of this contract.  Any references to pets or dogs in this contract shall refer to those specified on the Pet Information sheet(s).

Compensation

1) Where clients have purchased standalone consultations:
Behavet Ltd shall be paid the amount of £635 for the initial consultation (1.5-2 hours) and £435 for each follow-up consultation (45-60 minutes).
Insured clients are required to pay the necessary excess prior to the initial consultation. Clients for whom we are not making a direct insurance claim must have paid in full before the first consultation.
2) Where clients have opted for consultation packages:
Where consultation packages are purchased, the entire package price is required to be paid in full prior to the initial session, or if a payment plan has been selected, the first payment must be made in full before the first session. The price for the consultation package is as follows:
i) 6 month package: £1,602 (or £267 for 6 months)
3) Where clients have opted for a monthly support subscription:
Where clients subscribe to monthly support, the fee of £350 per calendar month is payable in advance. The first month's payment must be received in full before the initial session is booked. Thereafter, the subscription will renew automatically each month, and payment must be received prior to the next scheduled consultation or support being provided. The subscription will continue on a monthly rolling basis until terminated by either party in accordance with the terms of this agreement.

Consultations are not confirmed until payment is received.

Commencement
This Services Contract shall come into effect upon submission of this form.

Social Media
I give permission for Behavet Ltd to use photographs and video footage from our session/s for promotional purposes unless expressly said in writing prior to the consultation date.

Use of Personal Information

in regards to future bookings        /   upcoming courses        /   promotional material

Intellectual Property
PDFs, lesson plans, video links, and any other supplied materials remain the property of Behavet Ltd.  These are for private use of the client only and are not to be shared with any other person.

Fees, Cancellation and Early Termination

1) Follow-Up Consultations:
Should the Owner need to cancel a follow-up consultation, this should be done with as much notice as possible, and at least 7 working days before the consultation date. 
Cancellation with more than 7 working days notice will result in rescheduling of the session with no penalty.
Cancellations within 7 working days of the scheduled session will incur a £60 fee which will be charged to the card held on account.
Where Behavet Ltd needs to cancel a scheduled service due to unforeseen circumstances at short notice; they may appoint a substitute clinician with the approval of the Owner.  

Any wrongful or misleading information in the Primary Consultation Form may constitute a breach of this Services Contract and be grounds for instant termination thereof.  Termination under circumstances described above shall not entitle the Owner to any refunds or relief of any outstanding payments due.

2) Monthly Support Subscriptions:
For clients subscribed to monthly support, cancellation may be made at any time by providing a minimum of 30 days’ written notice. The subscription will terminate 30 days' from the date the notice is received. As payments are made in advance, the payment after completing the 30 day notice period will be cancelled, and services will remain available until the end of that 30 day period. All fees due during this notice period remain payable in full, regardless of whether services are used.

Liability
Behavet Ltd will carry professional indemnity insurance relative to the services performed for the Owner.  A copy of the insurance policy can be provided on request.
All training is undertaken at the owner’s own risk and they must assume full responsibility for themselves and their dog(s).  By attending these sessions and submitting this form, you waive all claims against Behavet Ltd and its clinicians individually or otherwise for any injuries, claims or damage that you, your dog or others might incur.

Whole Agreement
The Services Contract and Primary Consultation Form the sole and entire agreement.  Any alteration to this agreement must be in writing and signed by both parties.
